By requesting Smart Start Ignition Interlock Device (“IID”) removal;
I, the Smart Start client, understand that, to successfully complete my IID program (“program”), I must be in full compliance with all program requirements of Smart Start and the agency, entity or statute that required my participation in the program;
I understand that program violations can extend the amount of time that I must keep the device installed in my vehicle;
I understand that some agencies or entities will send me documentation confirming my eligibility to remove the IID, while others may require me to request the documentation and I may be required to provide such documentation to Smart Start prior to scheduling removal of the IID. I understand providing this documentation to Smart Start is solely my responsibility. The agency or entity should be able to help me in this regard;
I agree to pay Smart Start the fees required for removal along with any outstanding balances owed, prior to scheduling removal of the IID; and
Should I request removal of the IID without completing the program successfully and/or submitting the appropriate removal authorization, I understand that Smart Start is not responsible for early termination of the program and take complete responsibility for any consequences of unauthorized IID removal including, but not limited to, an installation fee if IID reinstallation is required.

You must take your Confirmation of Interlock form to your service scheduled appointments. When you have completed your required interlock period, you will submit the completed form to Driver Control.
If you have lost the form, you may request a Confirmation of Interlock Requirement Certificate, by selecting Next and following the steps indicated a representative will notify you of the next steps.

Upon servicing your device, the data will be sent to the State for review, allow 7-10 process business days to receive their final decision. Upon receiving your IID Compliance Certificate (removal authorization), you may submit it here and representative will notify you of the next steps.

Step 1: Contact us at (781) 871-7677 to inquire on your last six months of activity on the interlock device, in the event that you require documentation prior to your hearing - please note, documentation request processing time is 7-10 business days and will be emailed upon completion. If you have any records or receipts (vendor, mechanical or otherwise) related to infractions or violations that occurred within the last 6 months that you would like the Hearing Officer to consider; you may include these records with your Removal Hearing Sheet. Including these records may expedite your Removal Hearing.
Step 2: Go to https://atlas-myrmv.massdot.state.ma.us/myrmv/_/#1 In the first section listed under “MyRMV Online Service Center” click on “Reservations.”, select "Make or Cancel a Hearing Reservation". Click “I don’t have a letter ID” and click next. Follow the prompts from there.
Step 3: During the telephone hearing, the Ignition Interlock Department will review whether any infractions or violations occurred in the past 6 months. If the Hearing Officer finds actionable infractions or violations within the last 6 months, your Ignition Interlock Device Removal request will be denied. You will be required to participate in the IID program for an additional 6 months from the date of your last infraction or violation. If you do not have any infractions or violations, your Ignition Interlock Device will be approved for removal. The Hearing Officer will remove the Interlock restriction (T) from your license. Contact us at (781) 871-7677 to confirm approval eligibility on your vendor account.
Step 4: It is necessary that you obtain an unrestricted replacement Drivers License.</p.
Step 5: Once you have completed the unrestricted replacement Drivers License process, contact us us at (781) 871-7677 to have the device removed.

If you were required to have the IID on or prior to 11/30/2019, the State does not require you a compliance review to be submitted.

A2 restriction required, email us a copy of your DL to start the removal process to removals@smartstartinc.com.
A4 restriction required have your state authority submit your removal authorization directly to us or using our reporting portal, SmartWeb.
